Apply for an active kids & creative kids voucher
1. From 1 July, parents, guardians and carers can apply for
the second Active Kids voucher and creative kids voucher
2. Voucher is valid July to December
What you'll need
1. MyServiceNSW account
2.your child's Medicare card details
3.your child's height and weight (optional)
Who's eligible?

a NSW resident
-
aged between 4.5 and 18 years
-
enrolled in school (from Kindergarten to Year 12, including
-
those who are home-schooled or
-
enrolled in secondary school education at NSW)

a current Medicare card holder

Follow the prompts to apply for the voucher
